Getting Ready for Your Visit

In order for us to provide the best care for you, it is very important that you bring the following items to your visit:

  • A list of your current medications
  • Any and all x-rays, CT scans or Ultrasounds
  • Information about what treatments and medications have been tried in the past
  • Medical records from prior visits that you think may be helpful
  • Insurance information and card
  • Current credit or debit card
  • New Patients: download and fill out a registration and new patient forms below

If you cannot present your insurance card at the time of your visit, payment in full will be expected prior to you being seen.

Office Policies

Appointments / Office Hours

Appointments can be made by calling any of our office locations; Springdale: (514) 661-8800, Montgomery: (514) 682-7810, Eastgate: (514) 762-8122 or Lebanon: (514) 338-1976. Our general office hours are Monday-Thursday: 8 a.m. – 7:00 p.m. and Friday: 7:30 a.m. – 4 p.m.

Please note that early morning and evening appointments are not available at all offices everyday. Please click on the office above for further details.

Though every effort is made to adhere to our schedule, emergency cases always take precedence and will be seen as quickly as possible. We kindly ask for 24 hours notice if you need to cancel or reschedule your appointment.

After Hours and Emergency Care

If you are experiencing an emergency, dial 911 or head to the nearest emergency room. If you have a problem that requires immediate attention, please call any of our offices as soon as possible. If the office is closed, your call will be forwarded to the answering service, one of our providers will be contacted and will quickly call you back. If you have a service on your phone line that doesn’t allow you to receive “calls marked private”, please disconnect this service by dialing *87 prior to calling the doctor. This will allow the doctor to return your call. We do ask that calls after-hours be limited to emergencies only or a charge may be assessed.

Prescription Renewals

If you discover that your prescription will run out prior to your next visit, please call the office during normal office hours and have your pharmacist’s telephone number available. For your safety, we will only authorize refills after reviewing your records.

If you are using medications on a long term basis, including oral contraceptives, we may require an office visit more frequently than once per year. These visits assure your safety and optimal medical care.

Test Results

All of your laboratory results including pap smears, sonograms and mammograms are reviewed by your doctor. If an abnormality is found, you will be contacted as soon as possible. Please make sure your home and work telephone numbers are current. You may check to see if your pap smear was normal by calling LabCorp at 1-888-567-8723 about two weeks after your appointment. You will need to provide your Social Security Number, Date of Birth and the date the pap smear was obtained.

Financial

All office visits are payable at the time of service. We accept cash, checks, and most major credit cards and debit cards. You are expected to leave a credit or debit card number on file as part of our EasyPay program.

We believe our fees to be competitive and reasonable. We are providers for Humana, Anthem, UnitedHealthcare, Medical Mutual, Cigna and Aetna as well as most other major medical insurance companies.

Employer Required Forms

Due to the rising cost of healthcare and the increasing amount of time and complexity involved in filling out health related forms, a charge of $20 and 7-10 days will be required for us to provide this service to you.
